Patient Documents & their Timing (for Advocates)

The following is an overview of four key documents associated with a Patient's Solace journey, along with the point in the process at which each is generated and becomes available.

 

1) Physician Encounter

  • This is completed following the physician's intake and outlines key areas of support as a guide for the Advocate.
  • It populates in the Advocate's Patient File on the "Documents" tab.
  • The current Encounter is populated for the new advocate during Patient Transfer situations to ensure a seamless continuation of care.

 

2) Consent to Treatment / Patient Consent

  • This populates following the physician intake, after the encounter is submitted.
  • It lives in the Advocate's Patient File on the "Documents" tab.
  • It is electronically signed when consent is obtained during the initial intake, something providers are legally obligated to accept.



3) Letter of Authorization

  • This populates following the New Patient Visit (intro call), after the Advocate's Care Plan submission.
  • It lives in the Advocate's Patient File on the "Documents" tab.
  • It is electronically signed, something providers are legally obligated to accept.

 

4) Care Plan

  • Created by the Advocate following a New Patient Visit (intro call). It is reviewed and supported by the corresponding intake Physician.
  • This is a guideline of the patient's advocacy needs, based on the intro call conversation and the intake physician's encounter.
  • It lives in the Advocate's Patient File on the "Documents" tab.
  • The current Care Plan is made available to the new advocate during Patient Transfer situations to ensure a seamless continuation of care.
