The following is an overview of four key documents associated with a Patient's Solace journey, along with the point in the process at which each is generated and becomes available.
1) Physician Encounter
- This is completed following the physician's intake and outlines key areas of support as a guide for the Advocate.
- It populates in the Advocate's Patient File on the "Documents" tab.
- The current Encounter is populated for the new advocate during Patient Transfer situations to ensure a seamless continuation of care.
2) Consent to Treatment / Patient Consent
- This populates following the physician intake, after the encounter is submitted.
- It lives in the Advocate's Patient File on the "Documents" tab.
- It is electronically signed when consent is obtained during the initial intake, something providers are legally obligated to accept.
3) Letter of Authorization

4) Care Plan
