Care Plans

The Care Plan is a key part of the advocate-patient relationship. It is the document that captures the pertinent information obtained by the physician and advocate that is impacting the patient's health and well-being, both positively and negatively.

Care Plans consists of the following:

1. Patient Assessment
   • What brought the patient to Solace?
   • Patient Needs
   • Current Medical Status
   • Relevant Medical History
   • Care Team
   • Social Support System
   • Prior Care Management Experience
   • Care Barriers
2. Patient Goals
   • Your proposed actions
   • Education, coordination, support
   • Specific steps to help the patient meet objectives

Timing of Events

1. The New Patient Visit (intro call) is scheduled (following the completion of their physician intake visit).
2. The advocate-patient connection is established, and the advocate gains access to the intake physician’s encounter notes and can begin reviewing and prepping.
3. Following the intro call, the advocate will create a personalized Care Plan using the dashboard's template, outlining the patient's goals and a roadmap to achieve them.
4. Care Plans are submitted within 5 business days. These go to the referring physician for approval, but advocates should continue working with the patient in the meantime. Information will surface in the advocate's Patient File if further Care Plan attention is needed.

Care Plan Deadlines

• Submit a care plan within 5 business days of the first completed phone or video visit (a completed visit is over 15 minutes with both the patient and advocate present).
• No more than 5 hours of billable time can be submitted before a care plan is submitted.
• If a care plan is denied, resubmit within 5 days.

If these requirements are not met, timesheets cannot be submitted until the care plan is completed.
